chart biopsy
medical slang. a patient with a complex past medical history, usually in the era of paper charts instead of electronic records. the term describes the process of studying the old record to understand the current status of the patient.
mt. smith has 3 volumes of old medical records. i am going to have to do a chart biopsy to figure out what is going on with him.
